Upper Grand Family Health Team

A neighbourhood team of health professionals providing a full range of primary health care to the
residents of Centre and North Wellington, in the towns of Fergus, Elora, Arthur and surrounding areas.
The Upper Grand FHT includes doctors, nurse practitioners, nurse specialists, pharmacists,
dietitians, mental health therapists and social workers.

Please avoid coming to the office if you have a fever or cough. Call ahead if you need to come in and let us know if you have flu symptoms. Even if you are at risk, by taking precautions such as handwashing and wearing a mask, or booking your appointment at the end of the day we can still provide care.
